Common Name:

 Enter your agency’s 9 character ORI followed by a space and then your agency’s name.  For example:   OH0123400 ANYTOWN POLICE DEPARTMENT.  Please do not abbreviate.

Organizational Unit:

The default value should be BMV, do not change it.

Organization:

The default value should be Ohio Department of Public Safety, do not change it.

Telephone number:

Enter the number, beginning with area code.  Example: 419-222-1111.

Email Address:  

Enter the email address you use at your agency.  Certificate instructions will be sent to this email.

Cryptographic Service Provider

Choose Microsoft Enhanced Cryptographic Provider v1.0.  This is the default selection
