Choose the top choice to Make an End-Entity certificate request

All crash certificates requests will be verified with the Office of Criminal Justice Services and instructions will be sent via email to download and install the certificate into your computer.
The State of Ohio provides free crash software to law enforcement agencies for the purpose of completing crash reports and uploading them to the State. If you do not have the software or need training or other assistance with uploading crash reports please contact Tom Hollingsworth at 614-387-2800 or via email at thollingsworth@dps.state.oh.us.
The Department of Public Safety will also provide free GPS receivers to all agencies who are submitting crash reports electronically.
