____________, did while in a public place, to wit: (Location/Name of Public Place), refused to disclose his/her (name or address or date of birth), when requested to do so by a law enforcement officer, to wit: (Name of Requesting Officer and Official Title/badge number), who reasonably suspected (Name of Offender) was (committing, or had committed, or was about to commit) a criminal offense, to wit: (Criminal Offense and Code Section for Which There was Reasonable Suspicion), in violation of Section 2921.29(A)(1) of the Ohio Revised Code, a misdemeanor of the Fourth Degree.

 

 

Witness of felony offense of violence refuses to disclose name or address or DOB:

 

_____________, did while in a public place, to wit: (Location/Name of Public Place), refused to disclose his/her (name or address or date of birth), when requested to do so by a law enforcement officer, to wit: (Name of Requesting Officer and Official Title/badge number), who reasonably suspected (Name of Offender) had witnessed an offense of violence that would constitute a felony under the laws of this state, to wit: (Identify the Offense of Violence and Code Section that would Constitute a Felony), in violation of Section 2921.29(A)(2)(a) of the Ohio Revised Code, a misdemeanor of the Fourth Degree

 

 

Witness of felony offense of serious physical harm or property damage and refuses to disclose name or address or DOB:

 

____________, did while in a public place, to wit: (Location/Name of Public Place), refused to disclose his/her (name or address or date of birth), when requested to do so by a law enforcement officer, to wit: (Name of Requesting Officer and Official Title/badge number), who reasonably suspected (Name of Offender) had witnessed a felony offense that (caused or resulted in, or created a substantial risk of serious physical harm to another person or to property), to wit: (Identify the Felony Offense/Code Section and describe the risk of harm), in violation of Section 2921.29(A)(2)(b) of the Ohio Revised Code, a misdemeanor of the Fourth Degree.
